Requirements:

- NVQ Level 3 in a health/social care-related subject (or equivalent experience).
- Excellent communication and interpersonal skills.
- Ability to work both independently and as part of a multidisciplinary team.
- Computer literacy and good literacy skills are essential.
- A flexible approach to working hours is required as service needs.
- A valid driver’s license and reliable transportation are essential as this role involves travel.

About the Role:
The Congleton and Holmes Chapel District Nursing team is looking for an enthusiastic Health Care Assistant to join their friendly and supportive team. In this role, you’ll be providing essential care and support to patients in their own homes, working under the guidance of registered professionals. You’ll be making a real difference in people’s lives, assisting with personal care, medication administration, and other tasks that promote their health and wellbeing. It’s a chance to build meaningful relationships with patients and their families, empowering them to live more independently and comfortably. This is more than just a job; it’s an opportunity to be a part of something special and contribute to the modernization of community nursing services, in line with the NHS Long Term Plan.
Key Responsibilities:
- Providing direct patient care in their homes, under the supervision of a registered professional.
- Assisting with personal care, mobility, and other daily living activities.
- Administering medications and monitoring patients’ health.
- Supporting patients and their families with health education and self-care strategies.
- Working collaboratively with the multidisciplinary team to deliver the best possible care.
